Overview

Primary care may not represent a high percentage of the total cost of healthcare, but primary care practices influence almost everything else in the care stream. In this LIVE event, the leaders of Madison, Wis.–based Dean Clinic will discuss how they improved their primary care model through a radical redesign in only four years. Dean leaders will share how they evolved from the foundation of a patient-centered medical home to a more comprehensive primary care model using Lean tools and goals, system-wide metrics, a revolutionary new compensation model, data mining, care team redesign, and improved care coordination. This effort is enabling Dean to use primary care to drive down the total cost of care while improving quality and service.

Dean Clinic is one of the largest integrated healthcare delivery systems in the country. Established in 1904 and headquartered in Madison, Wis., Dean provides medical and health services through a network of clinics throughout southern Wisconsin; health insurance services through Dean Health Plan; ancillary health services within Dean Clinic locations; and clinical research and education through Dean Foundation. A privately held corporation, Dean has been a physician-owned and physician-governed organization since its inception. Ninety-five percent of Dean is owned by physician shareholders, while 5% is owned by SSM Health Care, a St. Louis–based order which also owns several hospitals nationwide.

2. Shift to primary care redesign and build internal Lean process improvement expertise

  • Expand PCMH into primary care redesign based on foundations of access, team care, and quality
  • Find waste and gaps in primary care with Lean value stream analysis
  • Tackle medication refill protocols, standardized PCP scheduling templates, previsit summaries, and a new walk-in triage

3. Refocus on care coordination and a consistent team-based care model

  • Define corporate metrics around people, quality, service, growth, and financial outcomes
  • Model standardized care teams that embody “right person, right work”
  • Balance enhanced care coordination services with care team mix

Speakers

John Butler
MD, Vice President of Medical Affairs

Dr. Butler joined Dean Clinic in 2001 as an Internist, later moving to St. Mary's Hospital where he served as the Chief of the Medical Staff before becoming Vice President of Medical Affairs. Returning to Dean as Vice President of Primary Care, he led the integration of the primary care practices of SMDV. Dr. Butler is a graduate of the University of Wisconsin-Madison and completed his Internal Medicine residency at the University of Cincinnati, and holds a Masters in Medical Management from Carnegie Mellon University.

Mark Kaufman
MD, Chief Medical Officer

Joining Dean Clinic in 1982 as a practicing general internist at the West Madison Clinic, Dr. Kaufman left full time practice in 1998 when he became the Chief Medical Officer for Dean Health Plan. He also served as Chairman of the Board of Dean Health System and as interim CEO prior to assuming the CMO role at Dean Clinic in 2007. Dr. Kaufman holds degrees from the University of Michigan and the University of Pennsylvania.

Michael McGrew
Process Improvement Team Lead

Michael McGrew joined Dean two years ago after working for UW Health and the University of Wisconsin Medical Foundation in Madison. He is responsible for implementing Dean's Clinic Improvement System and managing other Dean Clinic initiatives, including Primary Care Redesign. Mr. McGrew earned Bachelor's and Master's degrees in Industrial Engineering and an MBA from the University of Wisconsin-Madison.

Albert Musa
MD, Medical Director for Primary Care, East Region

After joining Dean in 1997 as a general internist, Dr. Musa held several administrative positions, including Chief of Staff at St. Mary's Hospital, before joining the Primary Care Redesign team in 2006. Dr. Musa is responsible for the Primary Care Medical Staff in Dean Clinic's East Region. He earned his undergraduate degree from Augustana College, received his M.D. from Rush Medical College, and completed residencies in Family Medicine and Internal Medicine at the University of Wisconsin-Madison. Dr. Musa has also achieved Six Sigma Black Belt Certification through the University of Wisconsin's Executive Education program.

Craig Samitt
MD, President and Chief Executive Officer

Dr. Samitt, is responsible for strategic planning of the health system, as well as the financial, operational and clinical performance of the company's 500+ doctors, 3000 staff, 60 facilities and health plan, retail and ancillary subsidiaries. Dr. Samitt previously held senior leadership roles at Harvard Vanguard Medical Associates, Harvard Pilgrim Health Care, and Fallon Clinic, Inc. He received his undergraduate degree from Tufts University, his Doctorate in Medicine from Columbia University, and his Master's in Business Administration from The Wharton School.

Steve Wilkes
Administrator for Primary Care, East Region

Mr. Wilkes joined Dean in 1990. He is responsible for day to day operations management and leading cultural and operational changes. He is also responsible for program development and design around local and system processes and innovation. A graduate of the University of Kansas, where he earned a Bachelor of Science Degree in Physical Therapy, Mr. Wilkes also holds a Master of Science Degree from the University of North Texas.
