HCFA memo: Don't use physician query form as med rec documentation
On January 22, the Health Care Financing Administration (HCFA) issued a memo to peer review organizations (PROs) that they are not to accept physician query forms as documentation in the medical record. PROs are to "educate and inform providers within their jurisdiction as necessary to ensure proper documentation."
