Designing proactive and reactive reporting systems
Consider this scenario: A nurse gives medication to a stable patient who then goes into immediate cardiac arrest and dies. On another floor of your facility, an operating room (OR) technician notices that the consent form and the OR sheet each indicates a different leg for the site of surgery, and informs the surgeon of the conflict prior to the start of the case.
Would your medical staff report either of these incidents to your risk management department or enter the information into your organizations medical error-reporting system?
