The team approach to documentation improvement is a good choice because it combines coder skills and modes of thinking with case manager connections and points of view. "Coders are a key component in a documentation improvement process because they already have a dialogue, in most cases, with physicians, clinicians, case management, and quality improvement," said Sheryl Spohn, RHIA, director of coding assurance at WellStar Health System in Atlanta.
Case managers bring to the table experience with the clinical team, said Teresa Fugate RN, BBA, CPHQ, CCM, manager in the North Carolina office of Pershing Yoakley & Associates. In other words, they can look beyond the documentation and enter the patient’s room to uncover the real situation. Fugate and Spohn discussed the coder-case manager model during a November HCPro audioconference, "The Coder/Case Manager Connection for Documentation Improvement."
