SPEAKERS:

Darius K. Joshi, MD, Director of the University of Michigan Health System Sub-Acute Care Rehabilitation Program, Joshi is a geriatric physician in the University of Michigan Health System Department of Internal Medicine Six years ago, he spearheaded U-M’s Sub-Acute Care Rehabilitation Program, which coordinates care between the hospital and care facilities to provide seamless and high-level transitions for hospitalized patients. Dr. Joshi received his medical degree from Grant Medical School in Bombay, India in 1989. He completed a residency at Wayne State University and a geriatric medicine fellowship at the U-M. Dr. Joshi’s primary clinical interest is the functional recovery of the elderly after hospitalization.

Teresa Riehle, RN, BSN, MBA, Director of Case Management for Mercy Health–Fairfield Hospital, Riehle manages Mercy Health's involvement in the Southwest Ohio Care Transitions Collaborative. Since 2007, Riehle has been responsible for Case Management, Utilization Management, Discharge Planning and Denials Management and has improved average length of stay consistently, exceeding benchmark goals. Previously, she held similar roles with Community Mercy Health Partners in Springfield, Ohio.
