When CMS released Transmittal 60, it made several modifications to its documentation policies that become effective on January 1, 2007. If your facility is not aware of the current documentation policies, it could be failing to document necessary services incorrectly and be risking denials.
Rehab providers are now required to file regular progress reports, and CMS’ rules stipulate strict limits on how to obtain signatures and who can write certain notes. The changes make a lot of the requirements that were once left up to individual states now national, forcing outpatient rehab providers around the country to adhere to the new rules to prove medical necessity and receive accurate reimbursement.

MEET THE SPEAKERS
Rick Gawenda, PT, is the director of physical medicine and rehabilitation at Detroit Receiving Hospital and the owner of Gawenda Seminars. Gawenda has worked in all areas of therapy within the hospital and home healthcare setting. He has provided education and consulting to hospitals, rehabilitation agencies, private practices, and his peers in the areas of CPT and ICD-9 coding, billing, documentation, reimbursement, and the appeals process for Medicare denied claims as they pertain to outpatient rehabilitation therapy services. He is the current President of the Michigan Association of Medical Program Rehabilitation Administrators, a member of the APTA, and the incoming President of the Health Policy & Administration section of the APTA. Rick is also on the editorial board for Briefings on Outpatient Rehab: Reimbursement and Regulations. He authored HCPro’s Coding and Billing for Outpatient Rehab Made Easy: Proper Use of CPT Codes, ICD-9 Codes, and Modifiers and The How-To Manual for Rehab Documentation.
Angela M. Phillips, PT, is President and Chief Executive Officer of Images & Associates. A graduate of the University of Pennsylvania, School of Allied Health Professions, Phillips has almost 30 years experience as a consultant, therapist, manager, educator, and hospital administrator, including extensive experience as a consultant for rehabilitation programs across the country.
