The most drastic change in DRG methodology since 1983 is coming your way.
CMS has proposed to implement a severity-adjusted DRG methodology effective October 1, 2007. The new inpatient prospective payment system (IPPS) rule is intended to align hospital payments more closely with the costs of patient care and the severity of a patient's conditions. CMS contracted with the RAND Corporation to evaluate five (5) alternative severity-adjusted systems that might be used by the PPS to classify discharges. However CMS moved forward with an Medicare Severity (MS-DRGs) system not previously introduced.
"It’s been predicted that when we move to a severity-adjusted system, coder productivity will go down by 30% and the need for documentation improvement programs will increase dramatically," says Deborah K Hale, CCS, president of Administrative Consultant Service. Since most case managers are involved with clinical documentation, they need to understand what the proposed model looks like and to start educating the hospital staff and physicians now.
