The Joint Commission’s 2008 National Patient Safety Goal 16A requires an appropriate response to any changes in a patient’s condition. MEET THE SPEAKERS
Della M. Lin, M.D., NPSF-AHA, is the chief executive director of continuing medical education at the Queen's Medical Center in Honolulu, HI and is an inaugural NPSF-HRET patient safety leadership fellow. Lin's physician leadership experience includes over 15 years work in credentialing, accreditation, peer review, surgical services team performance, education, and performance improvement. She is a frequent author, regional and national speaker in the field of patient safety and maintains an active practice in anesthesiology.
Glenn D. Krasker, MHSA, CHE, serves as president and CEO of Critical Management Solutions, a consulting firm based in Wilmington, DE. Krasker specializes in Joint Commission (formerly JCAHO) survey readiness; the turnaround of organizations faced with significant accreditation, licensure, and Medicare certification challenges; and in medical error risk reduction. He has worked in the health care industry for more than 25 years, serving in executive capacities for urban teaching and community hospitals. Krasker also served as director of the hospital accreditation program at The Joint Commission, where he was responsible for the accreditation of over 5,100 of the nation's hospitals. He has successfully coached many organizations through the process of regaining Joint Commission accreditation, achieving continuous survey readiness, and the challenge of investigating medical errors.
