MEET THE SPEAKERS
Patrick C. Mok, RPh, MBA is the Director of Pharmacy Services at Hoag Memorial Hospital Presbyterian in Newport Beach, California. He received his BS in Pharmacy degree from Massachusetts College of Pharmacy in Boston, Massachusetts in 1975 and a Masters of Business Administration degree Salem State College in 1985. He is a member of ASHP and CSHP. Patrick was the Associate Director of Pharmacy at Salem Hospital, Salem, Massachusetts until 1990 when he moved to California. He has been working at Hoag since 1990 and is holding the current position since 1997. Patrick has had many years of experience with Joint Commission standards after personally involved in more than 10 surveys. His current involvement is in Medication Safety initiatives and the implementation of a medication management module within a CPOE system.
Teresa Lee-Yu, PharmD, Director of Pharmacy, Providence Saint Joseph Medical Center. Teresa Lee-Yu has been the Director of Pharmacy at Providence Saint Joseph Medical Center, a 430 bed non-teaching community hospital in Burbank, CA since 1997. She received her Pharm degree from University of Southern California. She is a member of ASH.
In her role as the Director of Pharmacy, Teresa has participated in many patient safety activates, CMS Core Measures implementation/monitoring and PHI initiatives through out the hospital as well as with Providence Health and Service, a 26 hospitals not for profit health system headquartered in Seattle, WA. She is well versed in regulatory compliance standards and their applications in daily Pharmacy operation and implication in direct patient care.
