MEET THE SPEAKER
Jackie Birmingham, RN, BSN, MS, CMAC, is vice president of regulatory monitoring and clinical leadership at Curaspan Health Group, Inc. of Newton, MA. She has more than 25 years in discharge planning and case management experience including ten years as director of discharge planning in a large hospital, in home health and in information technology. She is the author of Discharge Planning: Tools for Compliance, Third Edition, published by HCPro.
Diane E. Holland, RN, PhD, is a nurse researcher and an assistant professor at the College of Medicine, at the Mayo Clinic in Rochester, MN. She has 20 years experience as a discharge planning coordinator and home healthcare agency manager. She has also developed several standardized discharge planning assessment tools.
