ABOUT THE SPEAKER:

Jackie Birmingham, RN, MS, CMAC, is vice president of regulatory monitoring and clinical leadership at Curaspan Health Group, Inc. of Newton, MA.  She has more than 25 years in transition management including discharge planning and case management. In her role at Curaspan Health Group, the company that builds secure patient-transition networks, Birmingham is responsible for regulatory monitoring to support compliance that impact transition management programs. She is a past-chair of the Legislative and Advocacy committee of the American Case Management Association, is certified in Case Management Administration Certification (CMAC). She is the author of several journal articles and books, including Discharge Planning: Tools for Compliance, 3rd Edition (2010) published by HCPro.

Karen Zander, RN, MS, CMAC, FAAN, is Principal and Co-owner of The Center for Case Management. Her pioneering work with clinical case management and CareMap® systems, began at New England Medical Center Hospitals in Boston, is internationally recognized. Hospitals and Health Networks has named her a “Cutting Edge” leader.
